Sliding fee scale payment

No patient is ever turned away for care due to an inability to pay at any Mansoor Pediatric locations. While we do accept most commercial insurances and Medicaid, we also offer payments on a sliding-fee scale. The sliding-fee scale is based on a patient’s household income and size. All you need is proof of income (if you are employed) and the number of people living with you. Proof of income could include: a most recent pay check, current 1040 tax return, unemployment check stub or worker’s comp check stub. If you are unemployed but your spouse works, please bring their most recent pay stub.
If you have no income at all, you will be asked to sign a form stating that you don’t have income. In some instances, patients using the sliding-fee scale can see a provider for as little as $10 plus the wholesale cost of lab fees. If an uninsured patient qualifies for Medicaid, clinic staff can help the patient apply for benefits right on site.
